1. Resident agrees that only the pet; described and named below will occupy the premises. No additional or different; pet is authorized under this agreement.

2. Resident agrees that pet will be kept inside the apartment at all times except; when on a leash and accompanied by and under control of the resident.

3. Resident agrees that if pet becomes annoying, bothersome, or in any way a nuisance to other residents or to the apartment operation, resident will, immediately upon notice from the owner remove the pet; from the premises or vacate the apartment.

4. Resident agrees to clean up any "litter" left by pet on grounds.

5. Resident agrees to deposit; with owner upon execution of this supplementary pet lease agreement the additional sum of $__________ paid by check, cash, or other. This deposit is separate and apart from the security deposit, and is refundable in full or part; after the move-out inspection, subject to conditions of the apartment due to pet damages. The pet deposit cannot be applied to rent by the resident and the resident's forwarding address must be left with the owners representative.

6. Resident agrees that pet will not exceed ______ inches in height and _______ pounds in weight when full grown.

7. Applicant must supply pictures of both sides of the animal for management files.

8. Resident agrees that if damages caused by the pet is in excess of $___________, that they are totally responsible for the full cost to repair or replace such damage done by the pet.
